This looks like an old problem with cocoon-servlet-service-impl and cocoon-core but has been
fixed a
few weeks ago. Are you sure that all of your SNAPSHOT dependencies has been rebuilt?

To answer your question directly: No I don't have any problem and I could run your test-case
using
mvn jetty:run directly from block. It displayed the page with CForm and parallel includes
successfully.
