When IncludeTransformer retrieves data from included sources, an error may occur.  In this
case, the error is propagated, but the transformation cannot be recovered until Cocoon is
restarted or the pipeline changes.  This is due to the fact that IncludeTransformer does not
remove the validity object when an error is thrown.

Note: when an error has occured, subsequent transformations return the <cinclude:include>
XML element as-is in the result of the transformation.
