Implements OptGroups in Static/Dynamic Selection lists for CForms

The patch implements optgroups for the current cforms trunk.
StaticSelectionList as well as the DynamicSelectionList now support it. Define
like this:

<fd:selection-list>
  <fd:item value=""><fd:label>mylabel</fd:label></fd:item>
  <fd:optgroup>
    <fd:label>grouplabel</fd:label>
    <fd:item value="1"><fd:label>subitem 1</fd:label></fd:item>
    <fd:item value="2"><fd:label>subitem 2</fd:label></fd:item>
  </fd:optgroup>
</fd:selection-list>

The forms-field-styling.xsl currently only renders optgroups and nested items
in  a dropdown list. Radio-/Checkboxes and output types are not implemented
yet.
