The problem is being caused by org.apache.cocoon.forms.util.DomHelper. In DomHelper.addLocalNSDeclarations()
expects elm.getAttributes() to return attributes with namespace declarations, which it doesn't.
This means that each instance of org.apache.cocoon.forms.binding.JXPathBindingBuilderBase.CommonAttributes.nsDeclarations
is always null.
