This problem is more general than just XMLByteStreamInterpreter, and can
potentially affect any AbstractXMLProducer on which only setContentHandler() has
been called.

I added a new o.a.c.xml.DefaultLexicalHandler (similar to
org.xml.sax.helpers.DefaultHandler) that is set as the initial lexical handler
in AbstractXMLProducer and thus ensures lexical events will always have a consumer.
