form-libs.js for CocoonForms uses the undefined key word.

Apparently Mac IE 5 doesn't recognize this.  A work around mention on the web
was to use the typeof operator and compare the result with the string "undefined"

so change

if (name == undefined) { ... }

to 

if ( typeof(name) == "undefined" ) {... }

I'll try and attach a patch once this bug is submitted.
