On further testing, patch does not fully work against current BRANCH_2_1_x as 
of this date.  It causes a parse exception when both a text input and a file
are sent in the same request.  

with:
curl -F foo=bar -F uploaded_file=@somefile.txt
http://localhost:8888/samples/blocks/xsp/java/upload 

and:
http://localhost:8888/samples/blocks/forms/upload

I get a org.apache.cocoon.servlet.multipart.MultipartException: Malformed header
       at org.apache.cocoon.servlet.multipart.MultipartParser.parsePart
(MultipartParser.java:179)
       at org.apache.cocoon.servlet.multipart.MultipartParser.parseMultiPart
(MultipartParser.java:131)
       at org.apache.cocoon.servlet.multipart.MultipartParser.getParts
(MultipartParser.java:104)
       at org.apache.cocoon.servlet.multipart.MultipartParser.getParts
(MultipartParser.java:110)
       at org.apache.cocoon.servlet.multipart.RequestFactory.getServletRequest
(RequestFactory.java:94)
