You do have to close those elements, like this...
         NOC_EQUIPMENT_TROUBLES_SEQ.NEXTVAL,
         '<sql:substitute-value sql:name="date"/>',
         '<sql:substitute-value sql:name="time"/>',
         '<sql:substitute-value sql:name="clli"/>',
         '<sql:substitute-value sql:name="remarks"/>'
Note the '/' characters added.
