XMLDB has only a stub implementation of exists().  Here is a first cut at a real
one.

A couple of notes.  1. This doesn't do anything with queries, but I think that's
the Right Thing: if the document exists, then it's OK to run a query on it, and
exists() should not care whether the result is null (any more than testing a
file for existence checks for empty files).

2. I return false if the there is an XMLDBException.  This is clearly wrong, but
I'm not sufficiently familiar with Cocoon internals to know what the right thing
is.  (Probably it's to raise an exception, and probably SourceException, but I'm
guessing here.)
