The problem described is a problem in 2.1 branch.
(I cann't verify how it is in 2.0.3 because with the web interface
I only have access to MAIN branch, isn't it?).
Look at AbstractTextSerializer.java from MAIN:

    protected TransformerHandler getTransformerHandler()
    throws javax.xml.transform.TransformerException {
        // return this.getTransformerFactory().newTransformerHandler();
        // FIXME - This is a workaround for bug #5779 of Xalan
        return this.getTransformerFactory().newTransformerHandler(
               new javax.xml.transform.stream.StreamSource(new
java.io.StringReader(org.apache.cocoon.xml.XMLUtils.xalanBugStylesheet)));
        // End workaround
    }

To solve the problem, I have to write:

        return this.getTransformerFactory().newTransformerHandler();
instead of
        return this.getTransformerFactory().newTransformerHandler(
               new javax.xml.transform.stream.StreamSource(new
java.io.StringReader(org.apache.cocoon.xml.XMLUtils.xalanBugStylesheet)));
