adds "failure-ok" for queries because failure is never acceptable but sometimes
you just don't care. 

The esql.xsl is really horrible.  I'm going to work on reformatting/refactoring
this bugger.  I don't claim to be a xsl brainiac but whoa...reading this thing
is like.....really hard.
