Scott,

1. Please add the following patch to sitemap.xmap. 
2. Please add enclosed HTML file as webapp/docs/samples/forms/add-department2.html
3. Specify the following in your JMeter Settings. (Choose Simple HTTP Tester)
   A. On Setup Page, point to http://localhost:8080/forms/add-department2.html
   B. On Setup Page, Timers Drop down combo box, choose random delay with gaussian distribution
   C. On Setup Page, Timers Average Delay, specify 1
   D. On Setup Page, Timers Average Deviation, specify 1
   E. On Run Page, Increase Thread Count to 12
   F. On Run Page, Actions Combo Box, Choose Spline Curve.
4. Now Choose Run Start and wait for sometime till the problem happens.
