Occasionally a VM will fail to migrate to a hypervisor for the following reason:

 Unable to migrate due to operation failed: domain
'i-2-122-VM' already exists with uuid a7da75fa-d79e-3838-82a0-589b9b317545

This can be fixed by running the following command on the target host:
# virsh undefine i-2-122-VM

I assume this is being caused because the undefine is not being run all the time. This only
happens to the odd VM, possibly after a hypervisor/host  restart.
