The same problem occurs in Sec. 4.5.7 (management-server-install-multi-node.xml)
If the Management Server is RHEL or CentOS, copy vhd-util to
/usr/lib64/cloud/common/scripts/vm/hypervisor/xenserver  ==> /usr/share/cloudstack-common/scripts/vm/hypervisor/xenserver

I will also suggest changing the next line, too:
If the Management Server is Ubuntu, copy vhd-util to /usr/lib/cloud/common/scripts/vm/hypervisor/xenserver/vhd-util
==> /usr/lib/cloud/common/scripts/vm/hypervisor/xenserver
This makes more sense, and will be consistent with the Ubuntu description in Sec. 4.5.3.3
