I just ran into it on 3.11.  Only 2 nodes showed the dead node in nodetool status. I ran the
usual 'nodetool removenode $UUID' and that hung. The logs on the other nodes showed that tokens
were moved around and replication happened, then nothing. I killed the nodetool command, ran
nodetool removenode force, and now everything is happy again.

to run *nodetool removenode $host-id*, give it about 10-15 minutes and then run *nodetool
