It appears this is exclusively csv files generated by Excel. Reproduction steps:
CREATE KEYSPACE test WITH replication = {'class': 'SimpleStrategy', 'replication_factor':
use test;
create table t (key text PRIMARY KEY, v text);
COPY t FROM '7639-repro.txt';
