    Delete use of entity constructors

    Replaces code (in tests) that uses entity-constructors directly. Deletes a few entity
constructors, now they are not used.
    
    This code is a step towards deleting more code from `AbstractEntity` that handles entities
being created directly via their constructors (deprecated since Brooklyn 0.5.0!). We first
need our tests to not be relying on that.

You can merge this pull request into a Git repository by running:

    $ git pull https://github.com/aledsage/brooklyn-server delete-entity-constructors-strike2

Alternatively you can review and apply these changes as the patch at:

    https://github.com/apache/brooklyn-server/pull/679.patch

To close this pull request, make a commit to your master/trunk branch
with (at least) the following in the commit message:
