When an element within an enclosing element is declared via

<element ref="prefix:type/>

in an xsd file with the elementFormDefault attribute set to "unqualified" the resulting xml
element node should contain the element's namespace prefix. Only those prefixes of inline
declared element (non-ref-Elements) should be omitted.

The implementation generates soap requests that do not validate against the xsd-schema because
it is omitting the prefix in both cases.
