Initial Analysis:
The RobustOperationClient is sending a message context without an envelope through the receiveFault.
MessageContext.isHeaderPresent NPE's because of the null envelope. This occurs because if,
in send(), the response envelope is null, a new one is created but not attached to the response
message context.
