carries on with an operation-specific OutFaultFlow (if there already is an operation context)
and then with the global out fault flow. Instead of just cancelling the in-flow, there should
be a mechanism that informs prior handlers of the in-flow about the occurrence of the exception,
so that they have a chance to undo their functionality. Axis 1.x had something like this (onFault
method).
