A brief note on how this change works

In SAAJ 1.2's perspective, adding XML declaration is up to SOAPMessage, not SOAPPart. Meanwhile,
Axis handles the job in SOAPPart. Moreover, SOAPPart.writeTo(Writer) is called by various
methods, some of which don't care about the XML declaration issue. With this change, XML declaration
is controlled by Axis Engine's generic configuration from WSDD and SAAJ SOAPMessage's specific
property. (Generic first, specific overridable)

To keep up with this refactoring, SAXOutputter handles XML declaration no more.

Finally, SerializationContext's sendXMLDecl is not referred at all practically. In other words,
sending XML declaration is not a SerializationContext's job. (I suggest cleaning it out.)
