A AxisFault throw in a early stage of invocation causes a NPE in SOAPFault.
The following call:
if (! cls.equals(AxisFault.class)) {
                FaultDesc faultDesc = 
                        context.getMessageContext().getOperation
().getFaultByClass(cls);

works only if a operation is set. In early invocation stages is this not 
guranteed.
I would suggest the attached patch.
