Add support for element that references a schema

Currently, the code ignores elements that references schemas.  For example:

<s:element ref="s:schema" />

It ignores these elements silently and thus when a response is received that contains the
schema an exception is thrown. 

I have chose to handle this by mapping schema references to the anyType and handle it as anyType.
 During my testing, I found a bug in the way we handle anyType in that namespaces declarations
on the element are thrown away.  For example, a response of 

<xs:schema id="NewDataSet" xmlns="" xmlns:xs="http://www.w3.org/2001/XMLSchema" >

would result in 

<xs:schema id="NewDataSet" >

This has also been corrected.
