I haven't checked the history, there may have been a time when mod_dav 
was using it directly; mod_dav-for-1.3 embedded a copy of SDBM. Not sure 
if there are any direct users remaining or whether it could be hidden in 
the next apr-util major version bump (or whether it's worth hiding if 
we're always going to build the code anyway).
