The shell currently has a constructor which takes an extra, unnecessary output stream parameter.
The ConsoleReader object already has an OutputStream associated with it, which we can use.
There is a comment in the shell to use that for the printing of the help when it became available.
I'm guessing that it did during the transition from jline1 to jline2.

We can drop the extra constructor parameter, and internally reuse the ConsoleReader's output
stream to print the help.
