The first item is a bug in the accumulo tracing code found by [~billie.rinaldi] and others.
It was a (dumb) optimization I put in to ignore spans that were shorter than a millisecond.
 Unfortunately, that leads to broken traces.  I probably should have put it into a separate
commit.  My bad.

I will add a log message about dropping traces, but I'm going to bound it by time, to avoid
spamming the logs.
